On Halloween night 1963, in the town of Haddonfield, Illinois, six-year-old Michael Myers, dressed in a clown costume, stabs his older sister Judith to death with a knife in their home. Fifteen years later, on October 30, 1978, Michael escapes Warren County's Smith's Grove Sanitarium, when his psychiatrist, Dr. Samuel Loomis, and his colleague, Marion Chambers, arrive to escort Michael to court, stealing Loomis' car in the process. Returning home to Haddonfield, Michael kills a mechanic for his overalls and steals a mask, knives, and rope from a local hardware store. The following day, on Halloween, Michael begins stalking high school student Laurie Strode after she drops off a key at his former house, so her father can sell it. Throughout the day, Laurie notices Michael following her, but her friends Annie Brackett and Lynda Van der Klok dismiss her concerns. Loomis arrives in Haddonfield in search of Michael, discovering that Judith Myers' headstone has been stolen from the local cemetery. He meets with Annie's father, Sheriff Leigh Brackett, and the two go to Michael's house, where Loomis attempts to warn the skeptical sheriff about the danger Michael poses. Sheriff Brackett patrols the streets while Loomis waits and watches the house, expecting Michael to return.
Later that night, Laurie babysits Tommy Doyle, while Annie babysits Lindsey Wallace just across the street, unaware that Michael has followed them. When Annie's boyfriend, Paul, calls her to come and pick him up, she takes Lindsey over to the Doyle house to spend the night with Laurie and Tommy. Annie is just about to leave in her car when Michael, who was hiding in the back seat, strangles her before slitting her throat. Soon after, Lynda and her boyfriend Bob Simms arrive at the Wallace house. After having sex, Bob goes downstairs to get a beer for Lynda, but Michael fatally stabs him. Michael then poses as Bob in a ghost costume and confronts Lynda, who teases him, to no effect. Annoyed, Lynda calls Laurie, but Michael strangles Lynda to death with the telephone wire just as Laurie picks up. Meanwhile, Loomis discovers the stolen car and begins searching the streets.
Suspicious, Laurie goes over to the Wallace house, and finds the bodies of Annie, Bob, and Lynda in an upstairs bedroom, as well as Judith Myers' headstone. Horrified, Laurie cowers in a hallway, when Michael suddenly appears and attacks her, slashing her arm. Barely escaping, Laurie races back to the Doyle house. Michael follows, but Laurie manages to fend him off long enough for Tommy and Lindsey to escape. Laurie defends herself by stabbing him with a knitting needle, a metal hanger, and his own knife, but Michael re-animates. Loomis sees the two children fleeing the house, and goes to investigate, finding Michael and Laurie fighting upstairs. Loomis shoots Michael six times, knocking him off the balcony; when Loomis goes to check Michael's body, he finds it missing. Loomis stares off into the night, while Laurie begins sobbing in terror. As various shots of the Doyle and Wallace houses are shown, a final shot of the Myers house is shown, as Michael's breathing is heard, indicating that he could be anywhere.

Laurie Strode comes to her final confrontation with Michael Myers, the masked figure who has haunted her since she narrowly escaped his killing spree on Halloween night four decades ago. In October 2018, forty years after the massacre in Haddonfield, true-crime podcasters Aaron Korey and Dana Haines travel to Warren County Smith's Grove Sanitarium to visit Michael Myers. They briefly interview Michael's psychiatrist Dr. Ranbir Sartain, a former student of the now-deceased Dr. Samuel Loomis, before meeting with Michael in hopes of gaining some insight into his past actions before his transfer to a new facility. Aaron brandishes Michael's mask at him, to no effect.
In Haddonfield, Illinois, Laurie Strode is living an isolated life, having been divorced twice, developed a strained relationship with her family and turned to alcohol. Laurie has prepared for Michael's potential return through combat training. The following night, Michael's transport crashes. Michael kills a father and his son who stumble upon the crashed bus on the road, and steals their car. The following day, on Halloween, Michael resumes his killing spree, killing several people at a gas station, including a clerk, mechanic for his overalls, as well as Aaron and Dana. He then recovers his mask and heads to Haddonfield. Laurie attempts to warn her daughter Karen, and her husband Ray, of Michael's getaway, but they dismiss her concerns.
Later that night, Allyson, Karen's daughter, finds her boyfriend Cameron Elam cheating on her at a school-sponsored Halloween dance, and leaves the party with his best friend, Oscar. Meanwhile, Michael kills two women. Allyson's best friend Vicky, babysits Julian Morrisey, when her boyfriend Dave arrives. Michael suddenly appears and attacks Vicky. Julian tries to save her but Michael fatally stabs her, when she sacrifices herself to protect Julian. Julian flees, as Michael kills Dave. Deputy Frank Hawkins, who arrested Michael in 1978, and Laurie hear the incident over the radio and head toward the house, and find the bodies of Vicky and Dave. Michael suddenly appears, and he and Laurie come face-to-face for the first time in four decades. Laurie shoots Michael who flees and she persuades Karen and Ray to seek protection in her heavily armed and guarded house. Michael comes across Allyson and Oscar, killing the latter. Both Hawkins and Sartain, who is now working with the police, arrive just in time to save Allyson. Hawkins subdues Michael, but Sartain, obsessed with Michael's enigmatic motivations, murders Hawkins before he can kill Michael. Hoping to see his patient in action, Michael instead kills Sartain, while Allyson flees. Michael kills two police officers and takes their car, arriving at Laurie's fortified house and killing Ray.
Laurie manages to get Karen to safety, before she engages in a showdown with Michael. Laurie severely injures Michael and severs two of his fingers, but he pushes her over a balcony; when Michael goes to check Laurie's body, he finds it missing. Allyson arrives and is reunited with her mother. Laurie, Karen and Allyson ambush Michael, trapping him inside the basement safe room, and Laurie sets the house ablaze. Laurie, Karen, and Allyson escape in the back of a passing pickup truck. As the house burns, a final shot of the burning basement is shown, with Michael nowhere to be seen. At the end of the credits, Michael's breathing is heard, indicating that he survived.
